Job summary

Three Oaks Hospice seeks a committed Registered Nurse to administer skilled nursing care to patients in their homes, coordinating with the interdisciplinary team and families to deliver compassionate, patient-centered hospice care.

The role requires an active RN license, CPR certification, and at least two years of clinical RN experience. Proficiency with Microsoft Office and strong communication skills are essential to support patients and caregivers in this specialized setting.

Qualifications

• RN license current in state of practice.
• CPR certification current.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Graduate degree not required; degree in Nursing must be from an accredited program.
• Proficient computer skills; experience with Microsoft Office preferred.
• Strong interpersonal, communication, and organizational skills; high ethics.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients.
• Coordinate care with IDT and family; manage plan of care.
• Educate patient/family on disease, end-of-life care, and self-care.
• Initiate appropriate nursing procedures and document progress.
• Coordinate services and referrals as needed; manage on-call duties.
