Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient using a computer; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.
• Demonstrate nursing practice with ethics and compliance.

Responsibilities

• Administer skilled nursing care for patients in their place of residence and coordinate with the interdisciplinary team.
• Coordinate care with patient/family and referring agencies; manage care plans.
• Initiate and revise the individualized plan of care (POC) as needed.
• Provide training to staff and educate patients and families.
• Prepare clinical notes and progress reports; document outcomes.
• Perform on-call responsibilities and respond to patient needs as assigned.
