Job summary

Heartland Home Health & Hospice LLC. is seeking a Registered Nurse to plan, organize and direct home-based hospice care, focusing on palliative care within patients’ homes and communities.

The RN will lead assessments, care planning, and education for patients and families, coordinating with the hospice team. The role requires active RN licensure in the state, CPR certification, and 2 years of acute care experience with hospice care preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in state.
• CPR certification.
• Hospice care experience preferred.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Complete initial and ongoing patient/family assessments.
• Provide professional nursing care per nursing process and state Nurse Practice Act.
• Develop and revise care plans with patient/family involvement.
• Administer medications and treatments as prescribed.
• Counsel patients and families on nursing and related needs.
• Coordinate patient care as Case Manager when assigned.
• Maintain accurate clinical notes and reports.
