Job summary

Heartland Hospice is seeking an experienced RN Case Manager to lead in-home end-of-life care, assess patients, and coordinate with an interdisciplinary team to ensure comfort and dignity for patients and families.

Full-time roles require an active Wisconsin RN license, hospice experience preferred, and a compassionate approach. Travel to patients' homes within Wisconsin is required, with competitive pay and benefits.

Qualifications

• Active RN license in Wisconsin.
• Hospice or palliative care experience preferred.
• Strong assessment and communication skills.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver nursing interventions and symptom management.
• Develop and update individualized plans of care.
• Coordinate care with physicians, nurses, social workers and other team members.
• Educate patients and caregivers on disease, self-care and end-of-life care.
• Document timely clinical records and ensure regulatory compliance.
• Support on-call and patient support activities as applicable.
