Job summary

Affinity Health Management is seeking a qualified Registered Nurse to plan, organize and direct hospice care in home and community settings. The role emphasizes community health education and experiences in nursing, tailored to meet the needs of patients and families.

The RN will conduct comprehensive assessments, develop care plans, administer medications, and coordinate with an interdisciplinary team to ensure compassionate, high-quality end-of-life care.

Qualifications

• Excellent observation, verbal and written communication skills.
• Problem solving abilities and basic math skills for nursing practice.
• Nursing skills per competency checklist.
• RN license in good standing required.

Responsibilities

• Assess patient and family to determine hospice needs; initial and ongoing assessments.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient/family involvement.
• Administer medications and treatments per physician's plan.
• Counsel patient and family; provide health care instructions; promote self-care and comfort.
• Coordinate patient care as case manager with interdisciplinary team.
• Participate in on-call duties and ensure equipment and services are available.
• Maintain accurate clinical notes and communications with physician and care team.
