Job summary

Heartland Hospice is seeking a skilled RN Case Manager to lead in-home hospice care, collaborating with physicians, LPNs, CNAs and social workers to deliver compassionate, personalized care. You will perform comprehensive assessments, develop plans of care, educate families, and ensure accurate documentation while upholding regulatory standards and quality improvement initiatives.

Qualifications

• Current Illinois nursing license.
• Minimum 1 year RN experience in hospice, home health, or related settings.
• Ability to work independently in patients' homes and community settings.

Responsibilities

• Perform comprehensive patient assessments and reassessments, educate patients and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care with visit frequency guidance.
• Coordinate care across interdisciplinary teams and with physicians.
• Document clinical records accurately and maintain infection control standards.
