Heartland Hospice is seeking an experienced Hospice RN / RN Case Manager to serve patients in their homes or preferred setting. You will act as the primary contact, deliver skilled nursing care, and coordinate with an interdisciplinary team to refine personalized care plans.

The role emphasizes compassionate, patient‑centered care, thorough assessment, and timely communication, supporting families through end‑of‑life processes and education.

Qualifications

• Current RN license in state of employment.
• Hospice or palliative care experience preferred.
• Strong communication and documentation skills.

Responsibilities

• Provide and document skilled nursing care per Plan of Care in patient homes or facilities.
• Assess patients regularly and adjust care plans accordingly.
• Collaborate with interdisciplinary team to develop personalized care plans.
• Educate patients and caregivers on disease, treatment, and end-of-life care.
