Job summary

Generations Hospice is seeking a compassionate RN Case Manager to join our team in Wisconsin. This full-time role supports patient-centered hospice care with a dedicated interdisciplinary team and after-hours triage.

We offer a $5,000 sign-on bonus, a strong patient-to-nurse ratio, and access to Homecare Homebase EMR for streamlined documentation. Travel across Winnebago & Fond du Lac Counties is required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
