Job summary

Gentiva Hospice is seeking an RN Case Manager to lead personalized hospice care in patients' homes, assisted living, or other settings. You will perform initial and ongoing assessments, deliver nursing interventions, and coordinate with physicians, CNAs, social workers, and chaplains to ensure compassionate, compliant care.

This full-time role requires RN licensure, CPR, and a minimum 1 year of RN experience, with Hospice or palliative care background preferred. A $5,000 sign-on bonus is offered.

Qualifications

• 1+ year RN experience in hospice, home health, or related setting.
• Hospice or palliative care experience preferred.
• Current RN license in state of employment and CPR certification.

Responsibilities

• Perform comprehensive RN assessments and ongoing reassessments.
• Deliver skilled nursing care per Plan of Care in home or facility settings.
• Collaborate with an interdisciplinary team to refine care plans.
• Educate patients and families on disease, care, and end-of-life options.
• Maintain accurate clinical documentation and comply with regulations.
