Job summary

Heartland Hospice in Independence, MO, is seeking an experienced RN Case Manager to lead hospice care for patients at home or in assisted living. You will assess patients, implement plans of care, and coordinate with an interdisciplinary team to ensure comfort and dignity.

The role emphasizes education for families, compassionate support, and timely documentation. Requires an active RN license, CPR, and a year of RN experience, with hospice/home health background preferred.

Qualifications

• Licensed RN with active state license.
• Minimum 1 year RN experience; hospice/home health preferred.
• CPR certification and valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings.
• Deliver skilled nursing interventions and document care per Plan of Care.
• Develop and update individualized plans of care with visit frequencies.
• Coordinate with IDG to refine personalized care plans.
• Educate patients and caregivers on disease, medications, end-of-life care.
