Job summary

Caretenders Hospice, part of LHC Group and the Optum family, is seeking a Registered Nurse Case Manager. You will oversee the Plan of Care for hospice patients, collaborate with the care team, and ensure compassionate, high-quality nursing care in patients' homes.

Requires Ohio RN license, 1+ year clinical hospice experience, and a valid driver’s license. Travel within a 50-mile radius, PRN scheduling with on-call evenings/weekends; comprehensive benefits and 401k offered.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license and access to dependable vehicle or public transport.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response.
• Collaborates with patient/family, attending physician and IDG daily.
• Instructs patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes regarding patient condition.
• Provide care in any home setting irrespective of age, race, creed, color, sex, disability, or financial condition.
