Job summary

Caretenders Hospice, part of LHC Group, is seeking a Registered Nurse Case Manager to deliver in-home hospice care in Ohio. You will manage the Plan of Care and collaborate with the IDG to ensure compassionate, evidence-based nursing for patients and families.

Requires an active RN license in Ohio, a valid driver’s license, and ability to travel within a 50-mile radius. Salary ranges from $79,600 to $94,000 annually, with standard hours and on-call rotation; PRN home care is also available.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or reliable public transportation.
• CPR certification required or ability to obtain within 90 days of hire.
• 1+ years of clinical experience preferred.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family responses to hospice care.
• Collaborates with the patient/family, attending physician, and IDG members in daily care.
• Instructs and supervises the patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient condition.
• Adapts to home situations regardless of age, race, creed, color, sex, disability, or financial condition.
