Job summary

Caretenders Hospice, a part of LHC Group and UnitedHealth Group family, is seeking a Registered Nurse Case Manager to provide in-home hospice care. This PRN/inconsistent position covers an approximately 50-mile service area in Ohio, with on-call evenings and weekends.

You will assess, plan, and implement care while collaborating with the care team to promote comfort and quality of life. Requirements include an active Ohio RN license, hospice/home health experience, and a valid driver’s license

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identifies physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with collaborative teams.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG for daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes and can work in diverse home settings.
