Ohio Living is seeking an RN Case Manager for Hospice to plan, coordinate, and deliver high-quality hospice care in patients' homes and care facilities across Ohio. The role ensures compliance with state and federal guidelines and focuses on compassionate, patient-centered care.

Must hold an Ohio RN license with at least one year of acute care or medical-surgical experience; hospice experience is preferred. Proficiency in EMR and Microsoft Office is required; weekend and on-call duties apply.

Qualifications

• RN license required in Ohio.
• One year experience in acute care or medical-surgical nursing required; hospice experience preferred.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint) and internet; EMR experience preferred.

Responsibilities

• Care Management and Service Delivery - 60%: provide ongoing assessment and nursing care to patients in homes and facilities; develop and implement care plans and coordinate with other health care providers.
• Quality and Compliance Management - 30%: audit and maintain patient records; participate in quality improvement projects; stay informed about regulatory requirements and third-party payers.
