Job summary

Caretenders Hospice in Columbus, Ohio, part of the Optum/LHC Group family, is seeking a Registered Nurse Case Manager to oversee hospice plans of care and coordinate with the IDG to deliver comprehensive home-based nursing.

The role requires an Ohio RN license, a valid driver's license, access to a vehicle, and willingness to travel within a 50-mile radius; standard hours are Mon–Fri, 8:00 AM–4:30 PM, with occasional on-call evenings and weekends.

Qualifications

• Current and unrestricted RN licensure in the state of practice (Ohio).
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Documents problems, goals, interventions, and patient/family responses.
• Collaborates with patient/family, attending physician and IDG for daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on the patient's condition.
• Provide care in home settings across a 50-mile coverage radius.
