Job summary

Gentiva is seeking a compassionate Hospice RN Case Manager to lead patient-centered care in patients' homes. You will assess, plan, and deliver skilled nursing services, coordinating with physicians, LPNs, CNAs, social workers, and chaplains to ensure comprehensive end-of-life support.

You will educate patients and families, manage symptoms, and maintain clinical documentation while upholding regulatory standards and quality initiatives across care settings.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related setting.
• Hospice or palliative care experience preferred.
• CPR certification.
• Valid driver’s license and auto insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with interdisciplinary team and physicians.
