Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead compassionate care at patients' homes or living facilities. You will conduct comprehensive assessments, implement symptom management, and coordinate with an interdisciplinary team to ensure high-quality end-of-life care.

Strong communication with patients, families, and providers is essential. Required an active RN license with hospice or home health experience; travel to patient residences is expected.

Qualifications

• Active RN license in state of employment.
• Hospice or palliative care experience preferred.
• Strong assessment and documentation skills.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized care plans with visit frequencies.
• Coordinate care with IDG members and physicians.
• Educate patients and caregivers on disease, end-of-life care, and medications.
• Document clinical records and maintain infection control standards.
