Job summary

Gentiva in Mobile, AL seeks an experienced Hospice RN / RN Case Manager to lead patient care in home environments. You will perform comprehensive assessments, develop personalized plans, and collaborate with an interdisciplinary team to ensure compassionate, high-quality end-of-life care.

The role emphasizes education for families, symptom management, and timely documentation, with on-call responsibilities as needed. A current RN license and CPR are required.

Qualifications

• Current license to practice nursing in the state of employment.
• Minimum of one year RN experience; hospice or palliative care preferred.
• CPR Certification required.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person/telephonic RN assessments and patient education.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patients and caregivers on disease, self-care, and end-of-life care.
