Job summary

Gentiva is seeking a Full-Time Hospice Field RN Case Manager to serve Jefferson County, Alabama. You will provide in‑home nursing care, manage plans of care, and collaborate with a diverse care team to support patients and families at end of life.

The role emphasizes education, compassionate communication, and adherence to regulatory standards while visiting patients in their homes, assisted living, or other living arrangements.

Qualifications

• Licensed RN in Alabama (or state of employment)
• Minimum 1 year RN experience in hospice, home health, or related field
• Ability to collaborate with an interdisciplinary team
• Strong patient and family communication skills

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments in home or facility settings
• Deliver skilled nursing care per Plan of Care and document care provided
• Develop and update plans of care with visit frequency
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains
• Provide education to patients, families and caregivers on disease, care needs, and bereavement support
• Maintain timely clinical records and comply with infection control standards
• Participate in on‑call rotations and team communication
