Job summary

Gentiva is hiring a Hospice RN / RN Case Manager to lead personalized end-of-life care in patients' homes or facilities. You will assess, treat and educate, coordinating with an interdisciplinary team including physicians, LPNs and social workers to maintain comfort and dignity for patients and families.

Requirements include an active RN license and CPR. A minimum of one year in hospice, palliative, or home health is preferred.

Qualifications

• RN required with state license
• Experience in hospice/home health preferred
• CPR certification required

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person/telephonic RN assessments and education for patients, families and caregivers.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care with visit frequency and risk identification.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and caregivers about disease, end-of-life care, medications and nutrition.
• Document timely clinical records, orders and progress notes; ensure infection control and compliance.
• Participate in on-call and support activities to ensure responsive hospice services.
