Job summary

Gentiva Hospice in Mississippi is seeking a compassionate Registered Nurse to serve as RN Case Manager, delivering personalized hospice care in patients' homes and related settings. You will perform initial and ongoing assessments, develop and update plans of care, and coordinate with physicians, social workers, and the broader care team to ensure comfort, dignity, and support for patients and families.

• Active RN license in Mississippi.
• Minimum 1 year RN experience in hospice, home health, or palliative care.
• CPR certification required.
• Valid driver's license and auto insurance.

Responsibilities

• Serve as primary contact for patients and families.
• Provide and document skilled nursing care per Plan of Care.
• Perform initial and ongoing patient assessments.
• Collaborate with IDG to refine care plans.
• Offer emotional, physical, and spiritual support to patients and families.
• Educate caregivers on safe patient care.
• Communicate condition updates to care team.
• Maintain clinical documentation and infection control compliance.
• Support scheduling and quality improvement initiatives.
• Navigate hospice topics like medications, hospice services, end-of-life.
