Gentiva Hospice is seeking a compassionate Hospice Registered Nurse Case Manager to lead patient care in home-based settings, coordinating with an interdisciplinary team to deliver personalized end-of-life care. You will perform thorough assessments, implement skilled nursing interventions, and educate patients and families on disease progress, medications, and care plans.

Formación

• Registered Nurse license required.
• Minimum 1 year RN experience (hospital, oncology, long-term care, hospice or home health).
• Hospice or palliative care experience preferred.
• Current driver’s license and auto insurance; CPR certification.

Responsabilidades

• Perform comprehensive patient assessments and reassessments, in person or via telecommunication.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with IDG and physicians; educate patients and caregivers.
• Document timely records and ensure infection control and regulatory compliance.
