Job summary

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead comprehensive, patient-centered home-based care. You will assess, implement, and adjust plans of care, coordinating with an interdisciplinary team to support patients and families through end-of-life processes with compassion and professionalism.

The role emphasizes nursing assessment, symptom management, education for caregivers, and thorough documentation to ensure quality and compliant hospice services.

Qualifications

• Current RN license in the state of employment
• Hospice/home health experience preferred
• Strong communication and teaching skills
• Ability to work in home-based settings

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings
• Deliver and document skilled nursing care per Plan of Care
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains
• Educate patients and caregivers on disease, self-care and end-of-life planning
• Maintain timely clinical records and ensure infection control and compliance
