Job summary

Gentiva seeks a Hospice RN / RN Case Manager to provide compassionate nursing care to patients in their homes or other settings. You will perform comprehensive assessments, develop plans of care, and coordinate with physicians, LPNs, CNAs, social workers, chaplains, and the broader team to deliver personalized end-of-life care.

Qualifications

• Active RN license in the state of employment.
• 1+ year RN experience in hospice, home health, or related areas preferred.
• CPR certification and valid driver's license required.

Responsibilities

• Perform comprehensive patient assessments and re Assessments, document findings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams with physicians and nurses.
• Educate patients and caregivers about disease, end-of-life care, medications, nutrition.
• Communicate condition changes to the care team for timely interventions.
• Maintain timely clinical records and ensure infection control compliance.
• Participate in on-call and patient support activities as applicable.
