Gentiva is seeking an experienced Registered Nurse for hospice and palliative care in patient homes or community settings. You will perform comprehensive RN assessments, deliver skilled nursing interventions, and coordinate with an interdisciplinary team to ensure compassionate, patient-centered care.

Responsibilities include creating and updating care plans, educating families, and ensuring timely documentation while adhering to regulatory standards.

Qualifications

• RN licensure in state of employment.
• Minimum 1 year RN experience; hospice/home health preferred.
• CPR certification and valid driver’s license required.

Responsabilités

• Perform comprehensive patient assessments and reassessments at patient home or facility.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate with IDG and physicians to refine care plans.
• Educate patients and caregivers on disease processes, medications, and end-of-life care.
