Gentiva Hospice seeks a dedicated Registered Nurse Case Manager to lead home-based hospice care. You will assess patients, implement plans of care, and coordinate with physicians and the IDG to ensure compassionate, comprehensive support for patients and families.

Responsibilities include education for patients and caregivers, timely documentation, and collaboration with an interdisciplinary team to optimize comfort and quality of life during end-of-life care.

Competenze

• Current license to practice nursing in the state of requested employment.
• CPR Certification.
• Current automobile insurance and valid driver’s license.

Mansioni

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per goals of care.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across interdisciplinary teams with physicians and nurses.
