Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead in-home, assisted living, or home-based hospice care. You will perform comprehensive assessments, deliver skilled nursing care, and create individualized plans of care in collaboration with physicians, LPNs, CNAs, social workers, and chaplains.

You will educate families, document care, and support end-of-life goals with compassion. You will coordinate with a full care team to ensure timely interventions, compliance, and high-quality

Qualifications

• Active RN license in Alabama or compact state.
• Hospice or palliative care experience preferred.
• Strong communication and education skills.

Responsibilities

• Perform comprehensive patient assessments and ongoing assessments.
• Deliver skilled nursing interventions per Plan of Care.
• Develop and update plans of care and visit frequency.
• Coordinate with interdisciplinary team for care plans.
• Educate patients, families on disease, end-of-life care, medications.
• Document clinical records and maintain infection control standards.
