Job summary

Elara Caring in Enid, OK is seeking a Registered Nurse Case Manager to assess patients, coordinate care, and educate families within the interdisciplinary Plan of Care. The role involves supporting hospice patients in various care settings and providing bereavement assistance as needed.

The ideal candidate holds an active RN license, has 1+ year clinical nursing experience, and is willing to travel between patient homes. Hospice or home health experience is preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes to the care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
