Elara Caring is seeking a Registered Nurse Case Manager to join our hospice/home health team. You will assess patients and families, coordinate care, and educate caregivers across various care settings.

The role requires an active RN license, 1+ year of clinical nursing experience, and the ability to travel between patient homes. A valid driver's license or reliable transportation is required.

Qualifications

• Current RN license is required.
• Graduate of an accredited nursing program (certificate, diploma, ADN, or BSN).
• 1+ year of nursing experience in a clinical care setting.
• Ability to lift up to 50 pounds and assist with patient transfers.
• Valid driver's license or reliable transportation to travel between patient homes.

Responsabilités

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
