Job summary

Elara Caring is seeking a Hospice Registered Nurse Case Manager to join our team and contribute to providing high-quality home-based hospice care. This full-time role requires compassionate patient care, clinical assessment, and collaboration with an interdisciplinary team.

You will educate families, support staff across care settings, and participate in bereavement activities. A valid RN license and reliable transportation are required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Ability to lift up to 50 pounds and assist with patient transfers.
• Must have a valid driver's license or reliable transportation for travel between patient homes.
• Experience in hospice or home health preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in the Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
