Elara Caring is seeking a Registered Nurse Case Manager in Tulsa, OK to assess hospice needs and coordinate care across settings. You will educate families and staff, monitor patient status, and support bereavement services as part of a compassionate care team.

The role requires an active RN license, a nursing degree, and at least 1 year of clinical nursing experience, with travel to patient homes and potential hospice/home health exposure.

Qualificações

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of nursing clinical care experience.
• Ability to lift, push, pull and assist up to 50 pounds.
• Must have a valid driver's license or reliable transportation for home visits.
• Hospice or home health experience preferred.

Responsabilidades

• Assess hospice patients and families to identify needs across physical, psychological, social, spiritual, and environmental domains.
• Monitor patient condition and promptly communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in the Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in care settings.
• Provide compassionate support at end-of-life and complete bereavement assessments for families.
