Job summary

Elara Caring is hiring a Registered Nurse Case Manager to join our nationwide home health team. You’ll assess hospice patients, coordinate an interdisciplinary plan of care, and educate families and facility staff while working in a home-based setting.

Qualifications

• Graduate of an accredited nursing program (RN)
• Current State RN license
• 1 year of clinical care experience
• Hospice/home health experience preferred
• 50% travel required
• Valid driver’s license and reliable transportation

Responsibilities

• Performs hospice/home health case management with emphasis on physical, psychosocial, and environmental needs
• Communicates findings and changes in health status to clinical team and physicians
• Instructs and counsels patients, families, and facilities in the plan of care
• Educates staff on hospice philosophy and pain/symptom management
• Provides bereavement support at time of death
