Job summary

Elara Caring in Oklahoma seeks a Registered Nurse Case Manager to lead hospice care across home settings. You will assess needs, coordinate care with physicians and the care team, and guide families through the interdisciplinary Plan of Care.

The role requires an active RN license, 1+ year of clinical nursing experience, ability to lift up to 50 pounds, and a valid driver's license for travel between patient homes.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate/Diploma/Associate/Bachelor).
• 1+ year of clinical nursing experience.
• Ability to lift up to 50 pounds and assist with transfers.
• Valid driver's license or reliable transportation to travel to patient homes.

Responsibilities

• Assess hospice patients and families for physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in the Plan of Care.
• Provide education on hospice philosophy, pain and symptom management to staff in various care settings.
• Provide compassionate support at death and complete bereavement assessments.
