Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join our team and support patients in home hospice settings. You will assess needs across physical, psychological, social, spiritual, and environmental domains and coordinate care with physicians and the care team to implement the Plan of Care.

The role requires an active RN license, 1+ year of clinical nursing experience, and reliable transportation for patient visits. A hospice or home health background is preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of nursing experience in a clinical setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must have reliable transportation to travel between patient homes.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
