Elara Caring is seeking a Registered Nurse Case Manager to join our hospice team and deliver High-Quality care in patients' homes. You will assess needs, monitor conditions, and coordinate with physicians and caregivers to implement the interdisciplinary Plan of Care.

Ideal candidates have an active RN license and at least 1 year of clinical nursing experience, with willingness to travel between patient homes; hospice/home health experience is preferred.

Formación

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program with 1+ year of clinical nursing experience.
• Ability to lift/push/pull up to 50 lbs and assist with patient transfers.
• Must have reliable transportation or access to travel between patient homes.
• Hospice or home health experience preferred.

Responsabilidades

• Assess hospice patients and families for physical, psychological, social, and environmental needs.
• Monitor patient condition and communicate changes to physicians and care team.
• Educate patients, families, and staff on the interdisciplinary Plan of Care.
• Provide hospice philosophy, pain and symptom management education to staff in care settings.
• Offer compassionate support at end-of-life and complete bereavement assessments.
