Job summary

Elara Caring is seeking a Registered Nurse Case Manager to continue our tradition of compassionate, high-quality care. You will assess hospice patients and families, monitor conditions, and support the care team with the interdisciplinary Plan of Care in patient homes and care facilities.

Your role includes educating staff on hospice principles, pain and symptom management, and providing bereavement support to families. Travel to patient locations and lifting requirements are part of the job.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of clinical nursing experience.
• Ability to lift up to 50 pounds and travel between patient homes.
• Hospice or home health experience preferred.

Responsibilities

• Assess hospice patients and families for holistic needs.
• Monitor patient condition and report changes to physician.
• Educate and support patients, families, caregivers, and staff in the Plan of Care.
• Provide education on hospice philosophy, pain and symptom management.
• Provide compassionate bereavement support to families.
