Job summary

Hospice of New Jersey is seeking a Full Time Hospice RN Case Manager (RNCM) to join our nursing team providing end-of-life care across Morris, Essex, Union, Passaic, Bergen and Hudson counties.

You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through every step of their hospice journey as part of an interdisciplinary team committed to compassion, clinical excellence, and connection.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.
