Job summary

Journey Hospice in Downingtown, PA is seeking a full-time RN Case Manager to oversee a primary caseload and provide skilled nursing care to patients and families under physician orders. You will collaborate with the Interdisciplinary Team to implement and adjust the plan of care.

The ideal candidate is a graduate of an accredited nursing program, licensed in PA, with hospice or home-health experience; CHPN certification preferred. A valid driver’s license and reliable transportation are required.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• PA RN Licensure in good standing.
• Valid driver's license and reliable transportation.

Responsibilities

• Oversee primary caseload of Hospice patients.
• Provide skilled nursing care per physician orders and agency policies.
• Conduct initial and ongoing nursing assessments.
• Teach, supervise, and counsel patients, families and caregivers.
• Collaborate with IDT to plan and revise care.
• Supervise Hospice Aides/Homemakers.
