Job summary

Hospice of New Jersey is seeking a Full Time Hospice RN Case Manager to join our compassionate nursing team, delivering hands-on hospice care and coordinating services across Morris, Essex, Union, Passaic, Bergen and Hudson counties.

The RN Case Manager will collaborate with the interdisciplinary team, educate patients and families, respond to referrals, and guide care plans to ensure comfort, dignity, and quality of life. Monday through Friday, 8 am–4:30 pm.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.
