Job summary

Gentiva Hospice is seeking a Registered Nurse Case Manager to lead patient care at home, coordinating with an interdisciplinary team to deliver skilled nursing and personalized plans of care. Remain compliant with regulations while educating patients and families about end-of-life care.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum of 1 year RN experience in hospice, home health, or related settings preferred.
• CPR certification as required.
• Ability to work independently in patient homes and coordinate with families.

Responsibilities

• Perform comprehensive RN assessments and reassessments with education for patients and caregivers.
• Deliver nursing interventions and symptom management per goals of care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care with physicians and IDG members.
• Provide education on disease, self-care, end-of-life care, medications, nutrition.
