Gentiva is seeking a compassionate Hospice RN / RN Case Manager to guide patients and families through the hospice journey in home or community settings. You will assess, plan, and document care, coordinate with physicians, LPNs, CNAs, social workers and chaplains, and provide emotional and spiritual support. On-call responsibilities and strict documentation are required.

The role emphasizes patient-centered care, regulatory compliance, and ongoing education for caregivers.

Formación

• RN with current license in applicable state
• Hospice or palliative care experience preferred
• Minimum of 1 year RN experience in acute care, hospice, home health or related setting

Responsabilidades

• Perform comprehensive patient assessments and ongoing reassessments
• Deliver and document skilled nursing care per individualized Plan of Care
• Coordinate care with physicians, LPNs, CNAs, social workers, chaplains and others
• Educate patients, families, and caregivers on disease, medications, and end-of-life care
• Maintain timely clinical records and ensure infection control compliance
• Participate in on-call services and care coordination activities
