Job summary

Ennoble Care is hiring a full-time Hospice RN Case Manager to serve patients in PA. The role involves managing caseloads, collaborating with an interdisciplinary team, and providing skilled nursing assessments and care planning.

A PA RN license and hospice experience are preferred, with a valid driver’s license and reliable vehicle required. You will document in EMR, coordinate with physicians and hospice staff, educate families on care levels, and participate in conferences.

Qualifications

• Valid State license to practice as a Registered Nurse in PA (or able to obtain)
• 1–2+ years of professional nursing experience, preferably in hospice care
• Ability to assess needs of patients and families and document comprehensively

Responsibilities

• Assess, plan, implement, and evaluate patient care across all nursing phases
• Document notes in EMR accurately and promptly
• Coordinate with physicians, hospice staff, and other agencies to ensure optimal care
• Educate families on care levels and breathing the patient’s physical, mental, and emotional needs
• Collaborate with clinical leadership to address issues and support professional growth
• Attend patient care conferences and participate in team discussions
• Supervise and document NAC every two weeks or as assigned
• Step in as associate nurse during off hours to support patient/family needs
• Provide support at time of death and bereavement period for families
• Participate in orientation and ongoing hospice training
• Maintain familiarity with agency policies and regulations governing hospice
