Job summary

Adoration Health in York, PA is seeking a compassionate Hospice RN to join our caring team. You will provide patient-centered, 1:1 care in the home setting, administer medications, implement care plans, and support families.

This on-site role emphasizes collaboration with an interdisciplinary team, documentation excellence, and participation in on-call rotations. A PA RN license and hospice experience are preferred; strong driving record required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with automobile insured and in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
