Job summary

Ennoble Care is hiring a Hospice RN Case Manager to reside within or near Chester County, PA. You will work with the interdisciplinary team to provide skilled nursing assessments, planning and care to maximize comfort for patients and families.

The role requires PA RN licensure, 1–2+ years nursing experience (preferably hospice) and strong collaboration with physicians and staff. Includes a flexible schedule and opportunities for leadership growth within a home-based care model.

Qualifications

Active Pennsylvania RN license.
1–2+ years nursing experience, preferably hospice.
Ability to assess and respond to patient and caregiver needs.
Team player in multidisciplinary settings.

Responsibilities

Manage assigned patient caseload within territory and perform assessment, planning, implementation and evaluation across nursing process stages.
Document notes in EMR accurately and timely.
Coordinate with physicians, hospice staff and other agencies to ensure optimal care.
Educate and coach on levels of care for physical, mental and emotional needs.
Participate in patient care conferences and discuss caseload issues with the team.
Provide support during off-hours as needed and assist during bereavement.
