Job summary

Ennoble Care is hiring a full‑time Hospice RN Case Manager who resides in or near Easton, PA. You will join an interdisciplinary team to provide skilled nursing assessments, care planning, and compassionate support for patients and families at the end of life in a home‑based setting.

The role emphasizes coordinating with physicians and hospice staff, documenting in EMR, and guiding families through levels of care while maintaining comfort and dignity for patients and their loved ones.

Qualifications

• Current PA RN license
• Experience in hospice care preferred
• Ability to assess needs of patients and families across settings

Responsibilities

• Manage assigned patient caseload within territory across all phases of nursing process
• Document all notes in EMR accurately and timely
• Coordinate care with physicians, hospice staff and other agencies
• Educate families on levels of care and care planning
• Participate in care conferences and interdisciplinary team discussions
• Support leadership in addressing issues and professional development
• Provide appropriate bereavement support to families
