Job summary

Journey Hospice is seeking a full-time Registered Nurse Case Manager in New Jersey to oversee a primary hospice caseload and provide expert nursing care to patients in home settings. You will perform nursing assessments, develop plans of care with the IDT, and educate families while coordinating with a dedicated care team.

The role requires NJ RN licensure, a BSN preferred, and CHPN preferred; valid driver’s license is required. Competitive salary and robust benefits offered.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• NJ RN Licensure by the NJ State Board of Nursing in good standing.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled professional nursing care to Hospice patients, families, and caregivers at home.
• Conduct initial and comprehensive nursing assessments and update as needed.
• Collaborate with the Interdisciplinary Team (IDT) to develop and revise the plan of care.
• Educate, supervise, and counsel patients, families, and staff on nursing care needs.
• Provide supervision to Hospice Aides/Homemakers to ensure high-quality care.
