Job summary
Virtua Health is seeking a Hospice RN Case Manager to join our hospice team in New Jersey. You will provide compassionate, home-based end-of-life care, coordinating with physicians and an interdisciplinary team to plan and implement personalized care for terminally ill patients and their families.
The role requires 2–3 years hospice nursing experience, NJ RN license, and CPR/BLS certification. On-call, weekend and holiday shifts may be required. Travel statewide is possible.
Qualifications
- Minimum of 2-3 years hospice nursing experience.
- BSN preferred.
- Current State of New Jersey nursing license to practice as a registered nurse.
- Current CPR certification and BLS.
- Must have current proof of valid driver's license and auto insurance.
Responsibilities
- Assess and document nursing process, including assessment, diagnosis, planning, implementation, and evaluation for home-based patients.
- Provide nursing care, emotional support and education to patients and families.
- Assist with wound care, treatments, procedures, and comfort care.
- Administer medications and treatments per plan to achieve comfort and symptom relief.
- Monitor signs and symptoms, document changes, and report to physicians and team.
