Job summary

Virtua Health is seeking a Hospice RN Case Manager to deliver compassionate end-of-life care in the patient’s home within New Jersey. Responsibilities include assessing comfort, educating families on symptom management, coordinating medications, and developing customized care plans with an interdisciplinary team.

The role requires 2-3 years hospice experience, NJ RN license, CPR/BLS, and a driver’s license. On-call, weekend, and holiday shifts may be required; travel statewide is expected.

Qualifications

• Minimum of 2-3 years of hospice nursing experience.
• Graduate of an accredited nursing program; NJ RN license required.
• CPR certification and BLS; valid driver's license and auto insurance.

Responsibilities

• Perform and document nursing process: assessment, diagnosis, planning, implementation, evaluation.
• Triage, identify, and act on changes in patient condition.
• Provide nursing care, emotional support and education to patients and families.
• Administers medications and treatments as prescribed to relieve symptoms.
• Collaborates with physicians and interdisciplinary team; participates in on-call, weekend and holiday coverage.
