Job summary

Serenity Hospice Care in New Jersey seeks an experienced RN Case Manager to deliver compassionate hospice care to patients and families. The role involves early assessments, ongoing care coordination, and collaboration with the healthcare team to ensure seamless, patient-centered support.

The RN will regularly assess nursing needs, develop and adjust care plans, and counsel patients and families regarding end-of-life care.

Qualifications

• Current NJ RN License in good standing.
• Hospice experience preferred.
• Willing to travel to assigned areas.
• Must have reliable transportation with liability insurance.
• Strong communication and interpersonal skills.

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluates the patients nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient and family, to provide continuous, well-coordinated care.
• RN counsels the patient and family in meeting end-of-life needs.
• Initiate appropriate preventative nursing procedures.
• Communicate with the physician regarding patient needs and changes.
• Supervise Hospice Aides and LPNs.
