Haven Home Health and Hospice is seeking a Hospice Registered Nurse Case Manager to deliver professional nursing care to home care patients under physician direction and agency policies. The RN may also work in institutional settings per policy.

The role includes admissions, supervisory visits, case management, interdisciplinary collaboration, and ongoing education. The position requires Oklahoma RN licensure, 2+ years hospice experience preferred, and strong communication skills.

Competenze

• Must hold a current Registered Nurse (RN) license in Oklahoma.
• Minimum two years hospice nursing experience preferred.
• Hospice and Palliative Nursing certification desirable.

Mansioni

• Provide direct nursing care to hospice patients across home and facility settings.
• Adhere to hospice plan of care and Medicare guidelines.
• Conduct admissions, routine skilled visits, evaluations, on-call duties, and PRN visits as assigned.
